Usage and Capacity Information Request Authorization

I authorize Eversource to provide the usage and capacity information for the account listed on this form.

OPTIONAL: In the event my load factor score does not meet the minimum % required to enroll in the Liberty Power Load Factor score plan now, but is at least %, please automatically enroll me in a Liberty Power electric supply rate program of cents/kWh for billing cycles with no early-cancellation fee starting with my meter reading date on or after March 1, 2021.

Customer Authorization*

I approve my enrollment for service. I have read, understand and agree to the Terms and Conditions for service. An electronic acceptance of the sales agreement is an agreement to initiate service and begin enrollment. I authorize Liberty Power to obtain and review information regarding my consumption history from my Utility. This information may be used by Liberty Power and any of its affiliates, agents and/or representatives to determine whether it will start and/or continue to provide energy supply service to me. This information shall not be disclosed to a third-party unless required by law. I authorize the release of information to Liberty Power. This authorization will remain in effect during the Initial Term and any Renewal Term of this agreement.*

Your energy supply will be provided by Liberty Power, and your energy delivery will continue to be provided by your utility. Your utility will still be responsible for delivering a reliable flow of electricity, and they’ll still respond to outages and emergencies. You can request a hard copy of this sales agreement. Liberty Power will send you a hard copy by mail within 3 business days after this request. You can rescind this sales agreement and authorization within 3 calendar days after electronic acceptance of the sales agreement, and you can rescind the authorization for release of information at any time.
